Cross Connection Control Portal Pre-registration

The CRD is committed to protecting your privacy. Your personal information is collected by the CRD under the authority of sections 26(c) and 26(e) of the Freedom of Information and Protection of Privacy Act (FOIPPA). This information is being collected for the purpose of creating a profile for you in the Cross Connection Control Portal and contacting you with instructions to complete your portal registration. This form collects identifying information and individual responses may be used to contact you or inform specific actions related to the Cross Connection Control Portal registration process and device testing requirements. Your information will be protected and handled in accordance with FOIPPA and used only for the stated purposes.
